The inscription in wreath, EVTVXWC TOIC KV
PIOIC, reads "Good luck to the emperors!", a reference to the elevation of Caracalla and Geta to Augustus and Caesar in 198 A.D.   The dual bust depicted on this medallion is a provincial dynastic issue as opposed to the more common denominational issue of other provincial dual bust coinage.

A nicer example of this medallion can be seen at: http://www.coinarchives.com/a/lotviewer.php?LotID=40106&AucID=43&Lot=630
